Patient Portal Access
The Patient Portal is set up to synchronize information between the patient and our electronic medical records system.
Once your account is set up, you can seamlessly update your information and contact our office through the web. We encourage you to use the portal to message our practice, request appointments, and communicate with your providers.

To Set Up Your Account
For patients who have used the previous Patient Portal (prior to November 2013), this portal will no longer be used for office communications.
If you already have your Security Code issued by our practice (on or after November 2013) and need to complete your registration, please click here.
If you do not have a Security Code, perform the following steps BEFORE your next visit:
- Request your Security Code by texting us through our HIPPA compliant texting sevice KLARA at 1 773-832-5246.
- After you receive your Security Code you may LOGIN

Please note, your visit may be delayed if you do not complete this information in advance. In order for you to gain access to the new Patient Portal to complete this information, your permission is required by law.
You may have a longer than usual wait time and delay during your next appointment if you do not complete this information in advance of your visit.
